111 Contact Code Vulnerable Consumer Application

Complete this form if you believe that you, or someone who normally lives at your premises, may qualify as a vulnerable consumer under the Commission 111 Contact Code.

Who may qualify?

A vulnerable consumer is someone who is at particular risk of requiring the 111 emergency service because of a specific health, safety or disability circumstance, and who does not have another means of contacting 111 at their premises that can operate for a continuous eight-hour period during a power failure.

If Hero accepts your application, we will work with you to determine an appropriate means of contacting 111 during a power failure. This will be supplied at no cost to you.
